the process of determining the underlying cause of a problem which might warrant corrective action. A root cause analysis is not a simple re-statement of what is wrong, but is a process of repeatedly asking probing questions about what went wrong, obtaining answers, and asking questions about the answers until the underlying cause of the problem is determined. Problems tend to recur unless the root cause is determined and corrected.
